Theory Summary and Why
The human factors theory of causation refers to the errors made by humans that lead to an accident. The three main human error factors that can cause an accident are overload, inappropriate response, and inappropriate activities. The work task is said to be overload when it exceeds the capability of a worker under environmental factor like distractions, internal factors like stress, and situational factors like unclear instructions. Knowing about the workplace hazard and still ignoring the safety guidelines refers to inappropriate response. Human error like inappropriate activities refers to the lack of safety training and misjudgment of the risk level. The reason why I agree with this theory is that it can accurately determine the chain of events, more likely a cause-and-effect relationship. The sequence of accident can be explained during the investigation. Workplace accidents resulted from human errors can be reduced in future by following the preventive measures.

Summary of Accident
On August 25, 2017, four gutter installation specialists were working to install gutters in a new private residence. According to the investigation done by Kentucky FACE, one of the workers was taking the measurements stepping on a ladder, which was positioned on the roof of porch. When he dropped the ladder, the base kicked out that made him unable to maintain the balance and fell from 10 ft. and 9 in. height landing on his head to the ground. His co-workers were not injured but, he was not responding after he fell. The victim was rushed to a local hospital. Unfortunately, he was declared dead on the same day. On further investigation, it is found that the lack of safety training and misjudgment of the risk resulted the incident. The workers ignored the safety guidelines while working at heights without any protection. The ladder was not placed on a leveled surface.
Relation Between Theory and Accident
The mentioned accident relates to human factors theory of causation because it holds a chain of human error that led to an accident. The workers ignored the hazards and safety measures which was an inappropriate response. The risk level was high when they used the ladder on the roof of porch. The investigation also concludes that there was a lack of safety training to perform a task in a safe and efficient way. They underestimated the degree of risk while working at heights. The incident reflects all three factors of human error: overload influenced by situational factor like risk level, inappropriate response to the hazards and safety rules, and inappropriate activities like lack of training and misjudging the risk involved at a given task.
